Indemnity Form

Family Camp 2026 • Required for all participants

To be completed by all those who attend camp. Parent/legal guardian must complete on behalf of children under 18. One indemnity form per family.

Bridgeway Family Church, CTMI and/or Konka, will not accept or incur any liability, damage or loss sustained by any person:

  1. Arising out of that person's presence at the Family Camp 2026;
  2. Arising out of the participation of that person in any activity whatsoever and in any form connected therewith; or
  3. Caused or occurred during or arising from any cause occurring during the period of that person's presence at the camp.

Whether caused or occurring in or at the camp venue or in any other place whatsoever and whether resulting from and/or contributed to by the negligence and/or other wrongful act of omission of the church and/or of any of its agents, or not caused or occurring as a result and/or in connection with the actions of any person in relation to any other animal.
